How many of you use model release forms and can anyone give examples of there own?

x2 - I generally use the AIPA forms unaltered, but occasionally hand-annotate or cross out pieces as suits the conditions of the shoot, just need to get both signatories to initial the change.

so, i wouldnt mind knowing a bit more about when a release form needs to be used?
and if i was in a busy street, would i need to get all 50 people to sign one if i wanted to sell the photo in the future?
i have found sites with plenty of info, but they are US, im not sure were to look for NZ info :(

As per smurff's previous question, I too am interested as to when permission needs to be granted. I took a photo recently at a public event - and am currently in the process of obtaining permission (trying to!) from those in the photograph just to cover my backside :) Was one of those photographs where you are taking tons of photographs and don't expect that you will want to use them in the public arena (as I now do!).
Is there are line that can be drawn that dictates when you require permission and when you do not?
